    Dear Sir  or  Madam,  
     
    How are you? Hope everything is fine with you. 

    I am a PhD student at University of Southern California and I am working on my dissertation focusing on spirituality and organizational behavior.I am writing to request your kind help in the process of collecting survey data for my dissertation. This research involving human subjects has been approved by USC institutional review board (IRB). 
     
    I have put the survey questionnaire on-line at the Survey Monkey website:

    If you are willing to complete this questionnaire yourself, that would be very helpful.  Just click on the above link and the online instructions will guide you through the task – it should take only about 15 minutes to respond to all the items. 

    More importantly,  I would greatly appreciate it if you would forward this request to other people who could also complete the questionnaire.  I would like to get data from a large, diverse sample of working adults, so your help in distributing this request to anyone over the age of 18 who is currently employed would significantly facilitate my efforts.
